Registration for the 2010 season is underway. Below you will find a list of the materials needed to register. Forms are available here.

Players must have:

A copy of your birth certificate or other accepted proof of age (such as a passport).
A complete Pop Warner Medical Release Form for the player's physical exam (in calendar year 2010) signed by a doctor, certifying that the player is physically fit to play tackle football. Do not wait on this one, set up the physical early.
A complete Player Contract signed by the player and parent/guardian.
A photograph, head and shoulders, NO HAT, 2"x2" or 2"x3".
A copy of your child's 2010 4th quarter report card.
Completed scholastic form.
Registration fee.
All Weigh-ins will be conducted by CDPW in accordance with the schedule. If you are unable to make the SSPW date, you should make arrangements to have your child weighed at a different CDPW site.
Paperwork Day is currently scheduled for July 17th, time to be determined later. A spreadsheet will be emailed out at a later date informing all people of their outstanding paperwork required for registration.

If you register online, you will still need to provide the following at a later date: medical release form, player contact form, 4th quarter report card, player photograph, and a completed scholastic form.
