2016 SSPW Registration for Cheer &

Football is NOW OPEN!!!!

The SSPW Board would like to welcome all our new and returning families to the 2016 Football and Cheer Season.

Below you will find a list of the materials needed to register. Some of the registration forms are available here.

Participants must have:

  • A copy of your birth certificate or other accepted proof of age (such as a passport). We will be keeping this copy.
  • A completed Pop Warner Medical Release Form for the player's physical exam (in calendar year 2016) signed by a doctor, certifying that the player is physically fit to play tackle football. Do not wait on this one, set up the physical early. If you are unable to get a physical due to insurance timing or appointment timing, you must get a note from the doctor saying that it is okay to play. Physicals are required for Football and Cheer.
  • A complete Player Contract signed by the player and parent/guardian.
  • A photograph, head and shoulders, NO HAT, 2"x2" or 2"x3".
  • A copy of your child's 2016 4th quarter report card. (You will need to submit in person at one of the designated registration dates.  See SSPW.net for dates/locations.
  • Registration fee and separate checks for all deposits. (SEE BELOW)
  • All initial football weigh-ins will be completed at registration to determine the team your child will be playing on.

COMPLETED REGISTRATION MUST BE RECEIVED NO LATER THAN THE FIRST DAY OF PRACTICE IN ORDER TO SECURE A SPOT

HOW DO THE DEPOSITS WORK

  • With respect to the Fundraising, you have the option of selling the Fundraising Tickets or not. If you turn in your tickets and money for the forty (20) tickets to the Fundraising Coordinator, your deposit check will be returned. If no tickets and money are submitted, your deposit check will be cashed.
  • With respect to the Concession Stand, your deposit check will be returned once two (2) hours have been worked in the Concession Stand.
  • With respect to the Equipment, your deposit check will be returned upon receipt of your equipment.
